In the U.S., 1 in 68 children is diagnosed with autism spectrum disorder (ASD). In fact, almost every school and university in the country has students with autism. Training Materials This care notebook template is designed to assist caregivers of children with special health care needs to maintain a record of their child's care, services, providers, and notes. Families and caregivers should bring their child's care notebook to all medical appointments, therapies, conferences, and vacations. The care notebook can be used to ensure that care is comprehensive, coordinated, and family-centered.
